What is Housing Stabilization Services

(HSS)?

At CareCrafters, our Housing Stabilization Services (HSS) are designed to help individuals with disabilities and seniors find and maintain stable housing. HSS includes Housing Focused Person Centered Plans, assistance with finding and securing housing, help with moving expenses, and ongoing support to ensure long-term housing stability. These services are crucial for those experiencing housing instability or transitioning from institutions, providing the necessary support to live independently and securely. Our goal is to ensure that every client has a safe and supportive living environment tailored to their specific needs.

Do I Qualify?

To qualify for Housing Stabilization Services (HSS) at Care Crafters, you must meet the following criteria:

  1. Medical Assistance (MA) Enrollment:

    • You must be enrolled in Minnesota's Medical Assistance (MA) program.

  2. Age Requirement:

    • You must be 18 years old or older.

  3. Documented Disability:

    • You must have a documented disability or disabling condition. This can include:

    • A physical disability

    • A mental health condition

    • A developmental disability

    • A substance use disorder

Need for Support:

You must require assistance in at least one of the following areas due to your disability:

  • Communication

  • Mobility

  • Decision-making

  • Managing challenging behaviors

Housing Instability:

  • You must be experiencing housing instability, which can include:

  • Being homeless or at risk of homelessness

  • Transitioning from an institution or licensed setting

  • Facing circumstances likely to cause homelessness (e.g., eviction notice, living in a condemned building)
