  • We offer three core services, each designed to address specific needs. Below is a breakdown of what each service includes and who it is for:

    1. Housing Stabilization Services (HSS)

    • What It Is: Support for individuals experiencing housing instability by assisting with finding and maintaining stable housing.

    • Who It’s For: Individuals with disabilities or mental health conditions who are homeless or at risk of losing housing.

    2. Home Healthcare Services

    • What It Is: Medical and personal care services provided in the home.

      • Home Care Nursing (Medicaid & Private Pay): Hourly nursing care for individuals requiring more support than shorter skilled nursing visits.

      • Private Pay Skilled Nursing Visits: Short Visits, focused nursing care for wound management, medication administration, and other supports.

      • Daily Living Services & Homemaking (Private Pay): Non-medical assistance, including personal care, light housekeeping, and meal preparation.

    • Who It’s For: Individuals who need medical or personal care at home due to chronic conditions, surgery recovery, or aging-related needs.

    3. Independent Living Services (245D Basic Support Services)

    • What It Is: Support services designed to help individuals with disabilities live independently.

    • Who It’s For: Individuals on waiver programs (CADI, BI, EW, DD) needing homemaking, IHS, and other basic support services.

  • Eligibility depends on the type of service and funding source. Currently, the services we offer that are covered by Medicaid include Home Care Nursing and Housing Stabilization Services (HSS). Other services are available through private pay or waiver programs.

    • Housing Stabilization Services (HSS) – You must have Medical Assistance (Medicaid), a disability or disabling condition, experience housing instability, and have an approved housing-focused support plan.

    • Home Care Nursing (Medicaid & Private Pay) – This service is covered by Medicaid for eligible clients and requires a nurse assessment. Private pay options are available for individuals who need longer nursing care at home. Private pay options are available for individuals who need longer nursing care at home.

    • Independent Living Services (245D) – You must be on a waiver program (CADI, BI, EW, DD) and receive a service authorization from a case manager.

    If you do not have Medicaid or a waiver but need care, we offer private pay options for Home Healthcare and Independent Living Services.

    • Referrals are for Medicaid/Waiver-covered services and must come from a case manager, care coordinator, or waiver provider.

    • Inquiries are for private pay clients or individuals interested in learning about services before obtaining a referral.

    If you are a waiver recipient, you can contact us directly, but you will need your case manager's information to proceed with a referral. If you are interested in private pay services, you can submit an inquiry using our Plan Your Home Care Services tool on our Home Healthcare page, and our team will follow up to discuss your care needs and budget.
